IT Change Request

Please provide the following information to request a change for an IT Serivce or Platform.

Change requests will be reviewed by the IT Technical Review Committee and implemented at the next available maintenance window when approved.

Emergency changes will be reviewed and implemented as quickly as possible.

A brief description of what is being done and the impact on the customer?
List any systems or services that will be affected by the change and what that impact will be on that system/services
What updates to knowledge base or Wiki pages will need to be done as a result of this change. If none, state 'None'.
Attach any documentation which may be relevant to the requested change
Files must be less than 2 MB.
Allowed file types: gif jpg jpeg png txt pdf doc docx xls xlsx.
List any other changes or tasks that must take place before this change can be implemented
List any performance indicators to be used to convey successful completion of proposed changes.
List any support teams who will be involved and what their involvement will be (i.e. Network Operations will be performing the upgrade. Once complete, report any outages to the IT Service desk, ithelp@louisiana.edu)
